Forms: Before a
student is allowed to practice or play in an East High sport, she/he must have a Physical examination on file - obtained in either
the 7th, 8th, or 9th grades are valid for participation through the 9th grade.
Any student who wants to participate in a sport in the 10th, 11th, and 12th
grade must have a physical examination form prior to participating in the first
sport = DATED AFTER JUNE 1 of their Sophomore Year. Physicals obtained in grade 10 are valid for 3 years.

_______ 1.Physical exam must be “NEW” for Grades 7-9
and 10-12. After 6/1.
_______ 2.Have not transferred schools.
_______ 3.Will not participate in more than six (6)
seasons in any sport in grades 7-12.
_______ 4.Have not accepted cash in any amount or
merchandise valued at more than $100 for participating in a sport.
_______ 5.Have not and will not compete in non-school
events in my sport after reporting for the school team.
